2010 - 2011 USAH Code of Conduct * - This is the USAH Code of Conduct Form. This form is used in conjunction with the CCYHA Code of Conduct and affirms our support of the USAH Zero Tollerance Policies. This Code of Conduct needs to be signed and is kept by each Team Manager in the Team Book.
2010 - 2011 USAH Consent To Treat * - This must be completed by each Skaters and returned to Team Managers/Coaches to be carried to each practice/game.
2010-2011 CCYHA Code Of Conduct * - This is the 2010-2011 Central Connecticut Youth Hockey Code of Conduct for Players, Parents and Coaches. This will be signed by all Players, Parents and Coaches and will be enforced by the CCYHA Board of Directors. This will be carried by each Team Manager and kept in the Team Book.
